Medicare

Pine Park Health accepts Original Medicare, with or without a Medicare Supplemental plan. We can also accept Tricare for Life and CHAMPVA as supplemental coverage.

Medicare Advantage Plans

We are in network for many Medicare Advantage PPO and HMO plans. In addition, we can often see PPO patients who have plans where we are out of network, though this may come with a higher out of network copay or coinsurance. If your plan is not listed below, call us at 1-888-512-1208 to learn more.

For certain services covered by your plan, you’ll pay a set amount — a copay. For other services, you’ll pay a percentage of the charges — a coinsurance. To know how much your copays or coinsurance are for services and prescriptions, please call the number on the back of your insurance card.

When covered under an HMO, you are typically required to have a primary care physician (PCP). To designate your PCP with your insurance plan, please call the number on the back of your insurance card. If you need help designating Pine Park as your PCP, please call 1-888-512-1208.
